Emancipating Youth Request Form

Ready to submit a request for emancipating youth?

Please fill out the form below and submitted for approval by Fostering Further. The reimbursement process usually takes at least 2 weeks.

Each youth may receive a maximum of $500 through this program. As the responsible organization, we need to make sure the money is going where it was intended to go. All requests will be made on a case by case basis, but could include:

  • Educational, Training, and Skills Expenses (e.g. school supplies, uniforms, course or lab fees, program fees, driver’s school, college entrance exams)
  • Apartment Furnishings (up to $200, typically through vouchers to Look Up Center)
  • Transportation Expenses (e.g. car, driving training, bike, bus pass)
  • Occupational Expenses (e.g. tools, required clothing)
  • Security Deposit
  • Therapeutic, Health & Safety Equipment (items recommended by therapist or to keep the youth safe)

Examples of things we do not sponsor include:

  • Memberships to organizations or recreational centers (e.g. zoos, science centers, YMCA, pools)
  • Items that are already being provided by another organization or agency (e.g. medical bills, baby formula by WIC, food, car seats, special occasion attire)
  • Day-to-day expenses (e.g. daycare, diapers, clothes)
  • Trips, vacations, or parties
  • Electronics for recreational use (e.g. video games, tablets)
  • Events or items that are more than 30 days in the past.

Please note:

  1. The request must be for a youth that has transitioned directly from foster care and involved with an Independent Living specialist or Bridges worker from Licking County, Ohio.
  2. Requester must be the youth’s caseworker or mentor.
  3. Once your request has been approved by Fostering Further, we may purchase the item from a business or provide reimbursement to the youth with a receipt from a business showing the purchase.
  4. There is a lifetime limit of $500.00 per youth.
  5. We always appreciate stories that tell how this request has impacted the youth’s life.
  6. We reserve the right to request recommendations or additional information, including from medical professionals or caseworkers.
  7. Even if your request is in line with all of the above guidelines, some requests may not be approved. It is the sole discretion of Fostering Further to determine if the community will be asked for additional resources to support a request.
  8. Certain information that you provide may be used on Facebook or other media, but any identification of the youth will be removed.
