Foster Parent Request Form

Ready to submit a request for your child in foster or kinship care?

Please fill out the form below, and if it is approved by Fostering Further and your agency, we will post it on our Grant a Wish page for the community to sponsor. The reimbursement process usually takes at least 2 weeks.

As the responsible organization, we need to make sure the money is going where it was intended to go. Examples of things we sponsor include:

  • Educational Expenses (e.g. school supplies, uniforms, or fees, course or lab fees, program fees, college entrance exams, class trips, and school pictures)
  • Graduation & Senior Expense (up to $150/youth)
  • Recreational & Socialization Expenses (e.g. sports, camps and trips organized by an outside program, such as 4H, band, church, or choir)
  • Skill Building & Self-Enhancement Expenses (e.g. music, scouting, and dance lessons)
  • Therapeutic, Health & Safety (e.g. items recommended by therapist, lice treatment, bed bug treatment)

Examples of things we do not sponsor include:

  • Memberships to organizations or recreational centers (e.g. zoos, science centers, YMCA, pools)
  • Items that are already being provided by another organization or agency (e.g. baby formula by WIC, preschool, car seats, special occasion attire)
  • Day-to-day expenses (e.g. daycare, rent, diapers, clothes, private school)
  • Family trips, vacations, or parties
  • Electronics for recreational use (e.g. video games, tablets)
  • Events or items that are more than 30 days in the past.

Please note:

  1. The request must be for a child currently in foster or kinship care (not yet adopted) in central Ohio.
  2. You must be the child’s current foster or kinship parent.
  3. Your agency will need to confirm that this is a current foster or kinship placement.
  4. Once your request has been approved by your agency and Fostering Further, we must have proof that you have purchased the item. You will be required to provide a receipt for reimbursement within 30 days.
  5. There is a limit of $500.00 per child per year, except for lice and bed bug treatments.
    1. Lice treatment is 50% of professional lice treatment up to $50 per child every 12 months. Does not include over the counter treatments.
    2. Bed bug treatment is 50% of professional treatment up to $500 per family every 12 months.
  6. We always appreciate stories that tell how this wish has impacted your child’s life. These can be sent from either the child or caregiver within 30 days of the reimbursement or event, and we welcome pictures that do not show the child’s face.
  7. We reserve the right to request recommendations or additional information, including from medical professionals or caseworkers.
  8. Even if your request is in line with all of the above guidelines, some requests may not be approved. It is the sole discretion of Fostering Further to determine if the community will be asked for additional resources to support a request.
  9. Certain information that you provide may be used on Facebook or other media, but any identification of the child or family will be removed.
